CPAP and your DOT physical: what examiners look for

The usage guideline commercial drivers are held to, and how to bring the right report.

If you hold a commercial driver's license and use CPAP for sleep apnea, your DOT physical will include a question every driver with sleep apnea knows: are you using it? The certified medical examiner wants proof that your treatment is working, and that proof comes from your machine's data.

The usage guideline examiners use

At least 4 hours a night, on at least 70% of nights, over at least 30 days.

There isn't a federal regulation that spells out a CPAP number. This guideline comes from advisory recommendations to the FMCSA and is what examiners commonly use. Your examiner makes the final call, and may ask for more.

What that means in practice

  • In a 30-day period, at least 21 nights need 4 hours or more of use.
  • A night with 3 hours 50 minutes doesn't count, even though you used the machine.
  • A night you didn't use it at all counts as a miss.
  • Nights you nap or sleep in your truck count too, so take the machine with you.

How much data to bring

  • New to CPAP: usually at least 30 days of use.
  • Renewing: examiners often want data covering the time since your last certification, which can be a full year.

Depending on your situation, an examiner may give a shorter certification (for example 90 days) while you build up a record, then a full year once your usage is consistent.

Making a report with CPAPGeeks

  1. Create a free account and choose United States as where you live.
  2. Open the SD card from your machine (choose the card itself, not a folder inside it).
  3. Click DOT compliance report, choose the period (30 days, 90 days, 12 months or your own dates), then Print or save as PDF.

The report shows your name, your machine and its serial number, the share of nights with 4+ hours, averages, and every day in the period, with a clear line saying whether the guideline is met. Check it against your machine's own usage screen before your exam.

Some examiners prefer a report from your equipment supplier or your machine maker's online system. Bring the CPAPGeeks report as well, and ask your supplier if your examiner needs theirs.

Tips for staying compliant on the road

  • Use the machine for every sleep, including naps in the cab.
  • Get a 12-volt power adapter or battery for your machine if your truck doesn't have an inverter.
  • If your mask is uncomfortable, fix it rather than skipping nights: see why masks leak.
  • Check your usage weekly, not the night before your physical. Small gaps are much easier to fix early.
